I acknowledge that I am aware of the risk inherent in water, including possible permanent disability or death and agree to assume all of those risks. I hereby waive any and all rights to claims for loss or damages caused by negligence or malpractice, active or passive of the physical therapist, ProAction Physical Therapy, Kids First Swim School, or of its staff. Also, I understand that there are no refunds for any missed classes/sessions.
