Worker’s Compensation

We work with patients who have been injured while at work. In order to process your claims appropriately, we need the following:

1. Date of your injury.
2. Workers Comp’s carrier name, claim number, telephone number and adjuster’s name.
3. Nurse case manager’s name and number (this is sometimes different from the adjuster)
4. Personal medical insurance.
5. Attorney information if one has been obtained.
6. Referring Physician’s name and contact information.
7. Prescription/Referral for Physical Therapy from your Physician.
8. If can be obtained, the medical diagnosis codes (ICD-10) from your referring doctor.
