COVID-19

All ProAction Physical Therapy staff have been fully vaccinated and received their booster shots.  We are requiring all patients to have had the 2nd Covid-19 vaccine within the last 6 months, or the more recent booster shot.  We will request a copy of your vaccination card as well.  If you have any questions, please contact us.
